Primary Care

Welcome to a better kind of care, where our team puts your healing needs first. We are proud to provide a high-quality level of customer service, medical experience, and commitment to health and wellness to all of our patients, right from the comfort of your home. Our goal is to make you feel better as quickly as possible and prevent hospitalizations due to complications with chronic conditions, and catastrophic events.

Our providers will assess you and create a custom preventative or recovery plan that is right for you. We will treat your existing conditions, but we also work to prevent pain and illness from occurring. We understand the importance of educating you on the most effective ways to take care of your body so that you can heal and prevent disease and catastrophic events.

Our providers treat the following:

Diabetes – Diabetes is a chronic (long-lasting) disease that affects how your body turns food into energy. There are three main types of diabetes: type 1, type 2, and gestational diabetes (diabetes while pregnant). More than 122 million Americans are living with diabetes (34.2 million) or prediabetes (88 million).

Hypertension (HTN) – High blood pressure increases the risk for heart disease and stroke, two leading causes of death for Americans. High blood pressure is also very common. Tens of millions of adults in the United States have high blood pressure, and many do not have it under control.

Congestive Heart Failure (CHF) – A chronic condition in which the heart doesn’t pump blood as well as it should.

Peripheral Artery Disease (PAD) – Peripheral arterial disease (PAD) in the legs or lower extremities is the narrowing or blockage of the vessels that carry blood from the heart to the legs. It is primarily caused by the buildup of fatty plaque in the arteries, which is called atherosclerosis. PAD can happen in any blood vessel, but it is more common in the legs than the arms.

Chronic Obstructive Pulmonary Disease (COPD) – Chronic obstructive pulmonary disease, or COPD, refers to a group of diseases that cause airflow blockage and breathing-related problems. It includes emphysema and chronic bronchitis.

Asthma – Asthma is a disease that affects your lungs. It is one of the most common long-term diseases of children, but adults can have asthma, too.

Dementia – Dementia is not a specific disease but is rather a general term for the impaired ability to remember, think, or make decisions that interferes with doing everyday activities. Alzheimer’s disease is the most common type of dementia. Though dementia mostly affects older adults, it is not a part of normal aging.
